Appointment Booking Instructions

New Patients: Please book an Initial Visit (30min) You can save money by booking a New Patient Package as your first visit.

Child (<18yo): Create an account for them and book the appointment under their name. Minors must be accompanied by a parent or legal guardian.

Returning Patients: schedule a 15min return visit.

Intake Forms

After scheduling an initial visit you will be emailed a link to fill out your intake form. Online intake form must be completed before your scheduled appointment.

Cancellation/No-Show Policy

If more than 24 hours you can reschedule yourself by logging in to your account. To notify less than 24 hour please call, text, or email the clinic. If less than 24 hour notice or you miss your appointment, a late cancellation/no-show fee in the full visit amount will be charged to the card on file.

Payment

Payment is provided directly by the patient. We accept all major cards, including FSA cards (flexible spending) & HSA cards (health spending). We can provide you with a super bill, which you may submit to your insurance provider for potential reimbursement. Reimbursement not guaranteed as it is dependent on your specific policy.

Insurance

The clinic is an “out-of-network” healthcare provider. We do not accept or bill any insurance, workers’ compensation, or personal injury cases. We are happy to refer you to another local chiropractor that specializes in those cases.

House-calls

House-calls are available throughout the week & cannot be scheduled online. Please call/text 415-818-2858 or email hello@shapechiropractic.com to schedule. House-calls are priced different than clinic visits.

Dr. Shane is passionate and effective at helping people achieve the healing results through natural healthcare methods. He has served patients of all ages from newborns to great-grandparents. In addition to working in his clinic, he has provided volunteer care at free health clinics in multiple countries around the globe.

Mastermind Certified in the Zone Technique by Dr. Peter Goldman, the founder of Zone School of Healing. Zone technique supports total body health & healing. Mastermind is the highest level of certification. Dr. Shane began mentoring with Dr. Goldman at his clinic in San Francisco, CA during his doctorate program. He continues his mentorship today.

Certified in the Webster Technique through the International Chiropractic Pediatric Association (ICPA) to support pregnancy.

Utilizes the Level Pelvis Method to support pregnancy, pelvis floor, and general pelvic health. (Developed by Dr. William J. Ruch)

Utilizes Advanced Adjusting Techniques (developed by Dr. William J. Ruch) providing gentle adjustments for individuals with high pain levels and limited mobility.

Dr. William J. Ruch was a teacher for Dr. Shane’s doctorate program. During the program Dr. Shane simultaneously learned from Dr. Ruch at his Oakland, CA clinic.
